These Terms of Service ("Terms") govern your access to and use of the websites, applications, and services operated by Patient Solutions LLC (doing business as "Any Hour Dental," "we," "us," or "our"). Any Hour Dental provides online dental connection services designed to help patients find and connect with dental providers. Our services include:
- Dental provider directory and search functionality
- Appointment scheduling and coordination
- Information about dental services and providers
- Emergency dental service connections
- Related dental health information and resources
Any Hour Dental acts as an intermediary to connect patients with dental providers. We do not provide medical or dental advice, diagnosis, or treatment.
3. User Accounts
3.1 Account Creation
To access certain features of our services, you may be required to create an account. You agree to:
- Provide accurate, current, and complete information during registration
- Maintain and promptly update your account information
- Maintain the security of your password and account
- Notify us immediately of any unauthorized use of your account
- Be responsible for all activities that occur under your account
3.2 Account Restrictions
Users may maintain only one account per site. Account creation must be performed manually without automation. Sales and transfers of accounts are strictly prohibited.
4. User Conduct and Prohibited Activities

8. Fees and Payment
8.1 Service Fees
Certain features of our services may require payment of fees. All fees are stated on our sites and are subject to change with notice.
8.2 Non-Refundable
Except as required by law or otherwise stated on our sites, all fees are non-refundable.
8.3 Payment Processing
Payment processing services are provided by third-party payment processors. By providing payment information, you authorize us to charge your payment method for all fees incurred.

11. Disclaimers and Limitations of Liability
11.1 "AS IS" Basis
YOUR ACCESS TO, USE OF, AND RELIANCE ON OUR SITES AND SERVICES IS ENTIRELY AT YOUR OWN RISK. Our services are provided on an "AS IS" and "AS AVAILABLE" basis without warranties of any kind, either express or implied, including but not limited to warranties of merchantability, fitness for a particular purpose, or non-infringement.
11.2 No Warranty
We do not warrant that:

- Any errors or defects will be corrected
- Our services are free from viruses or other harmful components
11.3 Limitation of Liability
TO THE MAXIMUM EXTENT PERMITTED BY LAW, PATIENT SOLUTIONS LLC SHALL NOT BE LIABLE FOR ANY INDIRECT, INCIDENTAL, SPECIAL, CONSEQUENTIAL, OR PUNITIVE DAMAGES, OR ANY LOSS OF PROFITS OR REVENUES, WHETHER INCURRED DIRECTLY OR INDIRECTLY, OR ANY LOSS OF DATA, USE, GOODWILL, OR OTHER INTANGIBLE LOSSES.
Our aggregate liability to you for all claims arising from or relating to these Terms or our services shall not exceed the greater of (a) the amounts paid by you to us in the six (6) months preceding the claim, or (b) $500.
12. Indemnification
You agree to indemnify, defend, and hold harmless Patient Solutions LLC, its affiliates, officers, directors, employees, agents, and licensors from and against any claims, liabilities, damages, losses, costs, expenses, or fees (including reasonable attorneys' fees) arising from:
- Your use of our services
- Your violation of these Terms
- Your violation of any rights of another party
- Your content or conduct on our services
13. Dispute Resolution
13.1 Informal Resolution
Before filing a claim, you agree to contact us and attempt to resolve the dispute informally for at least thirty (30) days. If the dispute is not resolved within thirty days, either party may pursue formal dispute resolution.
13.2 Binding Arbitration
Any dispute arising from or relating to these Terms or our services shall be resolved through binding arbitration administered by JAMS (Judicial Arbitration and Mediation Services) in Utah County, Utah, in accordance with JAMS Streamlined Arbitration Rules and Procedures.
13.3 Class Action Waiver
You agree that any arbitration or proceeding shall be conducted on an individual basis and not as a class action, consolidated action, or representative action.
14. Governing Law and Jurisdiction
These Terms shall be governed by and construed in accordance with the laws of the State of Utah, without regard to its conflict of law provisions.
To the extent arbitration does not apply, you agree to submit to the exclusive jurisdiction of the state and federal courts located in Utah County, Utah.
14.1 Limitation Period
Any claim or cause of action arising from or relating to these Terms or our services must be filed within one (1) year after the claim or cause of action arose, or it will be permanently barred.
15. Modifications to Terms
We reserve the right to modify these Terms at any time. We will provide notice of material changes by posting the updated Terms on our website and updating the "Last Updated" date.
Your continued use of our services after such modifications constitutes your acceptance of the updated Terms. If you do not agree to the modified Terms, you must stop using our services.
16. Termination
16.1 Termination by You
You may terminate your account at any time by contacting us or using account closure features in our services.
16.2 Termination by Us
We may suspend or terminate your access to our services at any time, with or without cause or notice, including for violations of these Terms.
16.3 Effect of Termination
Upon termination, your right to use our services will immediately cease. Provisions of these Terms that by their nature should survive termination shall survive, including but not limited to intellectual property rights, disclaimers, indemnification, and limitations of liability.
17. General Provisions
17.1 Entire Agreement
These Terms, together with our Privacy Policy, constitute the entire agreement between you and Patient Solutions LLC regarding our services.
17.2 Severability
If any provision of these Terms is found to be invalid or unenforceable, the remaining provisions shall remain in full force and effect.
17.3 Waiver
Our failure to enforce any provision of these Terms shall not constitute a waiver of that provision or any other provision.
17.4 Assignment
You may not assign or transfer these Terms or your rights hereunder without our prior written consent. We may assign these Terms without restriction.
